Resumo:1. A description is given of intradermal reaction of Berny and Mauze in cases of leprosy with extracts prepared from the urine of bacteriologically positive cases of leprosy. 2. Extracts prepared according to the method of Berny and Mouze from non-reacting bacteriologically positive cases of leprosy have not produced any reaction in all the lepromatous and most of the neural cases tested. Extract from the urine of one lepromatous case in the lepromatous cases tested. This however, was not a constant feature since extract from another reacting case produced reactions in only a few of the neural and none of the lepromatous cases tested (AU).

Resumo:1- Of ten patients, with various dermatoses, attending the outpatient clinic at New York Skin and Cancer Hospital, all reacted positively to tuberculin. Of this group 9 showed early and 8 late positive reactions to lepromin. 2- Of four patients with Boeck´s sarcoid, three reacted negatively to tuberculin and to lepromin. The fourth was weakly positive to tuberculin, negative to lepromin on the early reading but did develop a positive Mitsuda reaction. 3- Of 108 tuberculous patients at Seaview Hospital, 70.4 per cent were positive to lepromin on the early reading and 46.2 on the late reading. This high proportion of Fernández positives may be due to the fact that all individuals were tuberculin positives. If so, comparative figures for Mitsuda positives would indicate that the Mitsuda reaction is less affected by consensitization with m. tuberculosis than is the Fernández reaction. (AU).

Resumo:1- The diffusion activity (R factor) of human skin is 50 per cent that of bovine testicular extract. 2- The R factor is not modified in diseased and in apparently healthy skin of patients with tuberculoid leprosy. 3- In cases of lperomatous leprosy the diffusion activity disappears completely from the skin. 4- The diffusion activity ol leprous skin is inversely proportional to the amount of M. leprae it contains 5- Extract of lepromatous skin appears to exercise an antagonistic effect on the diffusion action of extract of tuberculoid skin. Resumo:The author describes the technique employed in the preparation of lepromin, the types of reaction found, the criterion of reading, etc. He considers as positive reactions those with more than 5 mm diameter in the 30th day of reading. Reactions were studied in 1529 individuals, both lepers and healthy, with or no contact with leprosy; and the results are correlated with age, sex, race, nationality, degree of bacillary elimination, types of diseases, debility of the body through various diseases, speed of sedimentation of the red cells, lepra reaction, (present or prior to the test). Analyzing his results, together with the works of various authors, about lepromin, in endemic and non-endemic zones, and comparing the data with those of general pathology, in particular with tuberculosis, he arrives at the following conclusions: Epidemiology - Leprosy is a highly contagious but also highly immunizating disease. The bacillary leper in the propagator of the infection. However, close or prolonged contact with such bacillary casesis not necessary to acquire the infection. Hence, in endemic countries a large part of the population have been contaminated and immunized. Infection occurs naturally at younger ages, not because of special reciptivity in infancy, but rather because adults were already contaminated in their turn. (The author makes a distinction in childhood between "real" anergy and anergy of "non-infection"). A predisposition to leprosy exists, which is represented by an incapacity to react with the... (AU).

Resumo:A skin test has been developed to determine the degree of competency in clearing bacilli from the tissues of patients suffering from various forms of leprosy. The test involves the intradermal ijection of a suspension of killed Mycobacterium leprae. The response of leprosy patients to the injection of other mycobacterial antigens, one prepared from M. lepraemurium and another from an atypical mycobacterium from a hamster, was also investigated in roder to study the isopathic phenomenon. Since lepromatous patients react negatively in tests with standard Mitsuda antigen, a concentration of 640 x 10(6) M. leprae per ml was used to producce macroscopic responses. The results of the test can be applied to determine the duration of consolidation treatment for lepromatous and indeterminate bacteriologically negative patients after regular treatment has ended. The test can also be used to indicate which Mitsuda-negative contacts should be given preventive treatment, and might be used to identify a given mycobacterium as M. leprae.(AU).

Resumo:The purpose of the work was comparison of reactivity of patients with the lepromatous and the tuberculoid type of leprosy. To 15 patients with the lepromatous and to 20 patients the tuberculoid type several antigens were administered intradermally. The antigens were obtained from alcohol - and acidfast microorganisms isolated from:- skin changes of cases lepromatous type, - skin of a case with tuberculoid type, - skin of a healthy person.The reactions to standard lepromin served as a reference. No correlation was found between the clinical picture, bacteriological examination and reactions to standard lepromin. A high percentage of positive results, observed in the group of cases with lepromatous type, using the antigen prepared from a patient with tuberculoid leprosy is of particular interest. This phenomenon may depend on differences within HLA subgroups in patients with the lepromatous and the tuberculoid type of leprosy. (AU).

Resumo:Although single-dose amphotericin B therapy appears to be immunostimulatory in mice, no data are available regarding the effects of chronic anti-fungal drug therapy on the immune system. We studied the effects on the guinea-pig cellular immune system of 4 weeks of treatment with amphotericin B, 5-fluorocytosine, or the combination of both drugs. The in vitro lymphocyte response to phytohaemagglutinin and the specific antigen, picryl human serum albumin (picHSA), were not affected by anti-fungal drug treatment. At 1.5 weeks of therapy with amphotericin B, skin test reactivity to picHSA was significantly reduced but returned toward normal by the end of 3.5 weeks of drug therapy. Macrophage migration inhibitory factor production by guinea-pig peripheral blood lymphocytes was significantly reduced after 4 weeks of amphotericin B therapy. No immunostimulatory properties could be ascribed to amphotericin B. 5-fluorocytosine had no effect on cellular immunity.(AU).

Resumo:Lepromatous leprosy (LL) patients whose bacillary load has decreased to almost undetectable levels by long-term chemotherapy failed to develop delayed-type hypersensitivity (DTH) to Mycobacterium leprae antigen following immunization with killed armadillo-derived m. leprae. When these LL patients were immunized with killed M. leprae in a mixture with live BCG, only DTH to purified protein derivative (PPD) was induced. These results are further evidence that immunological unresponsiveness to the leprosy antigen of patients with lepromatous leprosy is antigen-specific and non-reversible.(AU).

Resumo:The two groups of leprosy patients were compared with normal for the evaluation of the functional status of the autonomic nervous system, with the aid of several established procedures. Our findings reveal that in the leprosy patients the adrenergic system is hypo-functional as compared to the cholinergic. Further, in maculoanaesthetic group, there appears to be a physiological Dale's vasomoter reversal when adrenaline was used.(AU).

Resumo:Guinea pigs immunized with intact or disrupted armadillo-grown human Mycobacterium leprae administered in aqueous or oil vehicles were tested with various dilutions of M. leprae suspended in saline, water-soluble M. leprae extract, purified protein derivative, and a water-soluble extract of normal armadillo tissue. The results demonstrated the following. (i) Under no conditions was any skin test reactivity found to normal armadillo tissue extract. (ii) Positive sensitization to both M. leprae and its water-soluble extract was achieved by sensitizing guinea pigs with M. leprae suspended in Hanks solution or saline. Autoclaved M. leprae in Hanks solution or saline inoculated intradermally was an effective immunogen. Oil suspensions or emulsions were effective at sensitization, but appeared to be no better and, in general, slightly weaker, than simple inoculation in aqueous suspension. (iii) Living BCG failed to reveal a significant adjuvant effect on sensitization to M. leprae. However, cord factor appeared to potentiate slightly the sensitization to M. leprae in aqueous suspension. (iv) The minimum dose required for sensitization with M. leprae in aqueous suspension was 55 micrograms of purified bacilli. (v) Animals inoculated with M. leprae in saline or with M. leprae together with BCG showed positive skin test reactivity to the first skin test application made fully 1 year after the initial sensitization. The efficacy of autoclaved, irradiated M. leprae in aqueous, oil-free medium suggests a relatively safe approach to human vaccination studies.(AU).
